We are delighted to introduce this website, designed as a practical information resource for GPs and primary care professionals across North West London.
This website has been produced by the North West London CardioVascular and Stroke Network and the contributors are clinicians from your local Hospital Trusts with accredited stroke units and hyper-acute stroke units.
An A4 ring binder has also been produced and sent to every GP practice in NW London in September 2010. This accompanying website contains all the information featured in the guide as well as regular updates and referral forms available to download.
The new London stroke model was first introduced in February 2010, with a staged implementation reaching full operational capacity in July. We are sure that you would be interested to see how the London stroke model is performing and with this in mind, the first update ‘The story so far…’ can be found by clicking on the link in the left-hand toolbar. Regular updates will be added in the coming months and will be available to download from the ‘updates’ section of this website.
The progress so far demonstrates the value of this model and by increasing communication surrounding best practice we are confident that patient outcomes will continue to improve.
