In order to improve TIA patient pathways and to meet London’s performance criteria, new TIA referral forms have been created. Versions have been produced in EMIS, Vision, Locked Word and PDF format and are all available to download below.
The referral form should be completed as soon as possible and sent by fax to the preferred unit or clinic as outlined within the form.
Please note: the contact details (including fax numbers) of all TIA clinics across North West London are listed on the second page of the TIA referral form.
The form comprises:
- Patient and GP details
- FAST test
- ABCD2 Test
- Protocols for action to follow according to presentation and observations
- Weekday and out of hours service arrangements
- Contact details for all TIA clinics for weekdays and out of hours services
